(1) The license must be renewed in accordance with ARM 24.101.413 within 12 months of obtaining the previous license.

(2) Renewal notices will be sent as specified in ARM 24.101.414.

(3) The provisions of ARM 24.101.408 apply.

(4) A biennial physical examination shall be required after the license is granted. The biennial physical examination report, or proof of passing an applicable physical examination, shall be presented to the department every second renewal date after the original licensure date, and may not be dated more than 180 days prior to the renewal date of the license.
