(1) The department will assess the following fees:
(a) registered cardholder application fee of $75;
(b) provider application fee of $50;
(c) MIPP application fee of $50;
(d) a combined provider and MIPP application fee of $50; and
(e) annual registered cardholder renewal fee of $75.
(2) All fees must be submitted with the application and must be paid by check or money order payable to the Department of Public Health and Human Services.
(3) Fees are nonrefundable regardless of final application status.
(4) Renewal applications received by the department after the expiration date will be treated as new applications.
